Before attending summer camp, each camper must have an updated health history questionnaire on file at his or her camp location. Forms may be submitted electronically using the app below or download, complete, and turn in in-person at your camp's welcome center. 

Have your camper's immunization and health history ready before beginning the form. We recommend using a desktop computer when completing the HHQ and Camp Registration for the best experience.
