SURVEY: Health and health-related needs of Midland County

A survey is currently being administered to residents of Midland County.  The purpose of the survey is to understand the health and health-related needs of people living in Midland County.  Information from the survey will be used to develop programs to address the needs of its residents.  If you are 18 years of age or older, and live in Midland County, you are eligible to participate.  Your input is very valuable and needed.  Please click on the link to complete the survey.
